Use password to open Service Agreement form​

Your service contract is sent to you via email. You may also access it anytime in My Account.

For security reasons, your Service Agreement is password-protected.

When prompted, enter your password in the following format:​

For orders before 26 Oct 2023: <Identification Number><Birthday in DDMmmYYYY>​

​ E.g. If your birthday is 1 Jan 1998, your password would look like this: ​

Do note that the birth date has to be double digits and your password is case-sensitive e.g. the first letter of the month is in uppercase. ​

 

For orders after 26 Oct 2023: Identification Number (last 4 NRIC characters)><Birthday in DDMmmYYYY>​

Unable to key in the correct password​

As the password to access your Service Agreement form is made up of your personal information, log in to My Account with OnePass and review your details under Profile to check that it is correct.​
